*Renew Smiles Protection Plan covers: 1) Every 5 years, if a patient requires new dentures, Renew will cover 80% of the cost, and the patient will absorb 20% which is approximately a $1,000 out-of-pocket expense at current prices. 2) The implants are guaranteed for one year; 3) any adjustments are free for one year. To qualify for the Renew Smiles Protection Plan, Patients must follow all before and after surgery care instructions provided by Renew Smiles Affiliate Provider team members, including, but not limited to, the Renew Smiles Affiliate Provider post-operative protocol, as set forth in consents and other documents reviewed and signed by the patient prior to surgery. Failure to abide by these requirements and obligations, among others, will void the Renew Smiles Protection Plan. Normal wear and tear will not void the Renew Smiles Protection Plan provided that the Patient performs regular and appropriate maintenance of his or her dental implants and dentures, including, but not necessarily limited to, dental cleanings every six (6) months. Patients enrolled in the Renew Smiles Protection Plan program must submit certification of each such visit/cleaning to the Renew Smiles Affiliate Provider at which care and treatment were provided within thirty (30) days of service. The Renew Smiles Protection Plan will immediately be null and void if any dentist or other healthcare professional not affiliated with Renew Smiles, LLC performs any procedure of any kind, other than bi-yearly cleaning, on or involving (directly or indirectly) the dental implant and/or prosthetic denture. Neither Renew Smiles, LLC nor any of its affiliated provider dentists are liable for dentistry or services provided by or for any contracted or non-contracted third-party provider of any kind. Renew Smiles Protection Plan does not cover or apply to instances in which replacement or care is necessary based on injury to, or neglect on the part of, the patient or in instances in which there is damage resulting from trauma. The Renew Smiles Protection Plan is not available to a patient who presents for new dentures more than once during any given five (5) year period (the Renew Smiles Protection Plan may apply to the initial visit, but would not be available to any subsequent instance that occurs within five (5) years of that initial incident). If Patient’s treatment is financed by and through one of Renew Smiles, LLC, third-party lending partners (Renew Smiles, LLC is not a lender/lending institution), the Renew Smiles Protection Plan may, at Renew Smiles, LLC sole discretion, be voided if Patient/Borrower (if not the Patient), is in default under the terms of the agreement or contract with the lender, fails to remain current on his or her loan payment obligation, and/or is sixty (60) days or more behind in payment. Should the Patient/Borrower cure his or her default under the terms of the contract or agreement with the lender, Renew Smiles, LLC may, at its sole discretion, reinstate the Renew Smiles Protection Plan, subject to all the applicable terms and conditions. Renew Smiles, LLC may, in its sole discretion, change or alter the terms and conditions of the Renew Smiles Protection Plan.

Renew Smiles® procedure provider offices are independently owned and operated by licensed dentists in their respective states. A majority of patients qualify for same-day implants and can have their procedure in one day after initial workup, without additional bone graft surgery, and may leave the office that same day with provisional or final prostheses and, if necessary, the final prostheses to be placed later. Both general dentists and specialists perform the Renew Smiles® procedure. Dental specialists are specialists in oral maxillofacial surgery, periodontics, endodontics, pediatric dentistry, oral maxillofacial radiology, oral pathology, orthodontics, and prosthodontics. Dental implants are not recognized as a specialty. Dental implants can last a lifetime. Various clinical studies show that dental implants are likely to last at least 20 years with proper care and maintenance in most cases. Results may vary from case to case based on hygiene, medical history, and parafunctional habits.The maximum discount is $5,500 off retail fees. Not valid for previous or ongoing work. Cannot be combined with other discounts. Discount taken off usual and customary fees. Renew Smiles, LLC is not affiliated with any insurance provider and does not process insurance directly with insurance providers. If you choose to file a claim for reimbursement, you are solely responsible for all communication with your insurance company.
